TRANSPORTATION NEEDS: Non-modified vehicle
DIAGNOSES: Moderate ID, Down Syndrome, Hypothyroidism, Vitamin D Deficiency, High Cholesterol, Lactose Intolerant, Depression, and Arthritis.
HOUSING: Lives in congregate setting in Painesville
DAY PROGRAM/EMPLOYMENT: Individual attends ADS program, 5 days a week, M-F in Chardon (Brite Day).
