DESCRIPTION of REQUESTED SERVICES: The individual is a 16-year-old who will be moving into a new home soon in Lake County. He receives HPC services in his home. The team is looking for a wearable GPS device to assist in case of elopement. An inconspicuous option such as an in-sole tracker would probably be ideal.

TRANSPORTATION NEEDS: Non-Modified vehicle
DIAGNOSES: Autism Spectrum Disorder, Moderate Intellectual Disability, Macrocephaly, Expressive Speech and language development delays
HOUSING: Currently lives in community setting
DAY PROGRAM/EMPLOYMENT: Attends school
