DESCRIPTION of REQUESTED SERVICES: The individual is a 16-year-old who will be moving into a new home soon in Lake County. He receives HPC services in his home. The team is looking for ATQ threshold sensors to alert staff if the exterior exits are opened.

TRANSPORTATION NEEDS: Non-Modified vehicle
DIAGNOSES: Autism Spectrum Disorder, Moderate Intellectual Disability, Macrocephaly, Expressive Speech and language development delays
HOUSING: Currently lives in community setting
DAY PROGRAM/EMPLOYMENT: Attends school
