DESCRIPTION OF INDIVIDUAL: This individual is an 8-year-old male, enjoys his iPad and watching videos, has recently been dressing up in costumes (pirates, cop, etc.). He enjoys going on vacation and swimming in the pool. Individual may engage in physical aggression or self-injury when experiencing frustration, changes in routine or difficulty communicating their needs. Individual utilizes speech devices and uses some sign language as well.
PROVIDER REQUIREMENTS: Provider must be Med Cert 1, as well as certified in de-escalation/physical intervention.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Autism, ADHD, DMDD.
HOUSING: The individual lives in a home with his mother in Eastlake, Ohio.
DAY PROGRAM/EMPLOYMENT: Individual attends SAIL in Willoughby/Eastlake School District.
