DESCRIPTION of NEEDED SERVICES: 53-year-old female seeking HPC services in home 1x per week for 2-3 hours to assist with meals, medication and socialization. Individual would benefit from community integration but will need to become comfortable with staff inside of the home first. Days and times are flexible.

DESCRIPTION of INDIVIDUAL: The individual communicates verbally, but it is difficult for those who do not know her well to understand due to hearing loss and speech impediment. She enjoys watching tv/movies, talking with others, writing in her notebooks
PROVIDER REQUIREMENTS: Provider must be comfortable around large dogs.
DIAGNOSES: Down syndrome, Moderate ID, Bilateral Hearing Loss, GERD, Alopecia, Obesity.
HOUSING: Willowick
DAY PROGRAM:
SUPPORTS NEEDED: Assistance with communication with others, provide encouragement to expand her interest in varied activities, support needed to monitor Oxygen levels, meal prep, medication, hygiene care as needed.
