DESCRIPTION OF REQUESTED SERVICES:46-year-old male seeking looking for a HPC staff. Individual lives with mom in Madsion, Ohio Individual would need assistance with help doing his laundry and also everyday chores around the house. Individual would need help with some ADLS and hygiene with showering.

DESCRIPTION OF INDIVIDUAL: Individual is very laid back and is outgoing with everyone.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. It is recommended that a provider have training in a crisis intervention curriculum for support focusing on de-escalation.
TRANSPORTATION NEEDS: Individual requires no transportation
DIAGNOSES: Cerebral palsy
HOUSING: Individual currently lives with his mother in the family home in Madison
DAY PROGRAM/EMPLOYMENT: Individual currently works at YMCA
