DESCRIPTION OF INDIVIDUAL: Individual is a 28-year-old male who enjoys cutting the grass and gardening, helping his dad use the grill, watch videos of his favorite chef on his iPad, attending swim classes, and collecting/playing with toy garbage trucks.
PROVIDER REQUIREMENTS: Assistive technology consultation certification
TRANSPORTATION NEEDS: non-modified vehicle.
DIAGNOSES: Cerebral Palsy, Seizure diagnosis, left-side hemiplegia and left homonymous hemianopia (loss of vision in both eyes on the left side).
HOUSING: Lives in a family home with his mother and father.
DAY PROGRAM/EMPLOYMENT: Does not attend a program and is not employed.
