Services: Assistance with hygiene tasks (including assistance with enema), household cleaning, doctor appointment & community access.

TRANSPORTATION NEEDS: Non-Modified, able to fold wheelchair and transfer into regular seat
DIAGNOSES: Attention Deficit Hyperactivity Disorder (ADHD); Anxiety; Depression, Intermittent explosive disorder, CP-spastic diplegia, Seizure disorder of the absence type (controlled by medication), brain cyst, secondary Raynaud’s disease, Arthritis in toe on the right foot, GERD, Extropia, Asthma, Constipation, Carpal Tunnel
HOUSING: Lives in apartment with boyfriend
