DESCRIPTION OF INDIVIDUAL: Individual is a 30-year-old male. He lives at home with his parents and twin sister (also on a Waiver). He has been unsuccessful in day programs and needs more individualized services. Mom provides shared living but needs to lessen her work with him and get him engaged with others. He has been diagnosed with Autism and is nonverbal. He is ambulatory and spends a lot of time alone in his room. Family would like him to get out more.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-modified transportation
DIAGNOSES: Autism and Mild ID.
HOUSING: Individual lives with his two parents and sister in Painesville Township.
DAY PROGRAM/EMPLOYMENT: Individual is at home all day.
