DESCRIPTION OF NEEDED SERVICES: Seeking a provider for 40 hours of PDN a week. The hours have typically been 12 pm-10 pm at the individual’s home, Mon-Thurs. The family requests nursing providers with seizure experience. If interested, a 485 and NTI for PDN nursing services/hours must be completed to approve services. Nursing tasks include, but are not limited to, basic health assessment, neurological assessment, medication administration, including rescue medications as part of the seizure protocol and inhaled medications, post-ictal monitoring, suctioning, and bowel protocol.
SEEKING AGENCY OR INDPENDENT PROVIDER: The family would entertain both Agency and Independent providers

DESCRIPTION OF INDIVIDUAL: Independently mobile with a history of elopement. History of physical aggression towards herself and her caretakers. The individual has an enclosed bed, door locks, and cameras, which are to be used only by natural support/non-paid providers.
PROVIDER REQUIREMENTS: Provider must be a PDN-certified provider, either an RN or LPN
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: The individual is a 9-year-old female who lives in Mentor. She has complex medical, neurological, developmental, feeding, and visual needs that require ongoing specialty care and continuous caregiver support. Her primary diagnoses include GRIN2B-related neurodevelopmental disorder (loss-of-function), drug-resistant, uncontrolled Lennox-Gastaut syndrome/network epilepsy with an increased risk of Sudden Unexpected Death in Epilepsy (SUDEP), hypotonic cerebral palsy, cortical visual impairment, chronic pediatric feeding disorder with aspiration risk, intellectual disability, anxiety, and incontinence.
HOUSING: The services will be provided at the individual’s home in Mentor, OH.
