DESCRIPTION OF NEEDED SERVICES: 25-year-old female who resides in Painesville with her parents looking for HPC three times a week (no preferred days) from around 4 pm-6 pm to provide engagement in community activities. Need assistance with safety in community, crossing streets, and assistance with any purchases.

DESCRIPTION OF INDIVIDUAL: Friendly (shy at first), sweet, female who enjoys recreation outings, going to movies, shopping, out to eat, outdoor concerts in community, festivals. She is bilingual in English and Spanish.
PROVIDER REQUIREMENTS: Ideal experience working with individuals with mental health/dual diagnosis. Individual has times where she experiences an increase in symptoms related to mental health diagnosis.
TRANSPORTATION NEEDS: non-modified vehicle.
DIAGNOSES: Autism, Moderate Intellectual Disability, Schizoaffective Disorder, Anxiety
HOUSING: Individual resides with her mother/father in Painesville.
DAY PROGRAM/EMPLOYMENT: Individual attends Vocational Hab during the day Monday-Friday.
