DESCRIPTION of INDIVIDUAL: PROVIDER REQUIREMENTS: This person is a 34-year-old male with a good sense of humor. He enjoys Pluto TV and Power Rangers. He resides by himself in Eastlake. Female staff only!
Monday-Friday
Assist with cleaning, cooking and shopping. He would like to find a provider willing to assist him to comic cons in other states within driving distance. He last went this summer to Virginia. Provider would need a reliable vehicle and be able to drive a small RV.
A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Monosomy-18
HOUSING: Lives alone in single family home
