DESCRIPTION OF NEEDED SERVICES: Female looking for a congregate setting.

DESCRIPTION OF INDIVIDUAL: She is 38 years old; she benefits from advance notice and prompt when routines change. Staff assist with medical appointments, medication administration, and monitoring for illness or behavioral changes. Staff must be Med Certified 1. She communicates preferences by saying “no” or through gestures such as tapping. Staff should monitor anxiety triggers, provide reassurance, and use calm, simple communication. When upset, redirect her to preferred activities and involve familiar staff when possible. She prefers her personal space, and for others not to stand directly behind her. She is working toward greater independence with daily living skills. She needs assistance following a BM and with hygiene, meal preparation, household chores, laundry, and room maintenance. She can use a microwave with prompts but requires assistance in preparing meals. Food must be cut into pieces, and staff must monitor meals, prompting her to eat slowly and drink between bites. She enjoys regular contact with her parents. Staff support family relationships, social opportunities, decision-making, self-advocacy, and positive rapport. She requires ongoing auditory supervision at home/day program and visual supervision in the community, with total assistance during emergencies. Staff assist with transportation, phone calls, community participation, and recreation. During vehicle rides, she benefits from holding a preferred item. She requires total assistance with money management, budgeting, banking, bills, benefits, and wage reporting. Requires monitoring of physical well-being, including overheating and allergic reactions, and follow nursing protocols as needed.

PROVIDER REQUIREMENTS: Staff assist with medical appointments, medication administration, and monitoring for illness or behavioral changes. Staff must be Med Certified 1.

TRANSPORTATION NEEDS: non-modified vehicle.

DIAGNOSES: Diagnosed with profound intellectual disability, global developmental delay, autism spectrum disorder, and anxiety disorder.

HOUSING: Looking for congregate setting in Lake County. Currently living in mentor.

DAY PROGRAM/EMPLOYMENT: Not at this time
