DESCRIPTION OF REQUESTED SERVICES: Individual is a 26-year-old male seeking Residential Respite Services, not to exceed a total of 52 hours/month. Individual has no alone time. He requires visual supervision at home, close and constant supervision in the community.

SEEKING AGENCY OR INDEPENDENT PROVIDER:  Open to either Independent or Agency Provider.

DESCRIPTION OF INDIVIDUAL: This individual is verbal but can be hard to understand at times due to his enunciation of words. He can be very shy and tends only to speak to people that he knows very well and feels comfortable with. He is determined and knows his likes and dislikes. He can be silly and joke around. He enjoys watching cartoons on YouTube, playing video games (Super Monkey Balls Two) and being on the computer. Enjoys taking pictures or videos on a tablet. He likes vacuuming and spraying spray bottles. He can be sexually inappropriate with exercise balls and bean bag chairs. He will elope at times especially when in a new environment. Rarely, he will throw items when he is frustrated. When driving in the car, he may try to open the door or attempt to throw items out the window. These behaviors typically occur when he desires attention, is fixated on an item, and wants to escape a task. Individual struggles with understanding redirection from his family or staff, He needs physical assistance with bathing, brushing his teeth, wiping after a bm, hair care, and nail care. Requires total assistance in diabetes management including monitoring carb intake, checking glucose levels, monitoring for Catatonia and insulin administration.

PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. The provider would need to have experience with individuals with behaviors and be familiar with de-escalation techniques and redirection. The Provider would need to be med. Cert. 3 to assist in diabetes management, insulin administration.

TRANSPORTATION NEEDS: non modified vehicle.

DIAGNOSES:  moderate intellectual disability, autism, catatonic disorder, anxiety, type 1 diabetes (insulin dependent diabetes mellitus-brittle) and OCD.

HOUSING: Individual resides in his Family home with his parents in Madison
