DESCRIPTION OF NEEDED SERVICES: Individual is seeking vocational habilitation supports part-time seeking 1-2 days per week of attendance.  Individual is willing to increase his days of support after receiving time to transition to his new program.  Individual would not need NMT transportation to and from program.

DESCRIPTION OF INDIVIDUAL: Individual enjoys cleaning and helping others.  He works best with having clear and concise instructions of tasks provided to him.  He prefers to work on one task at a time and have a clear understanding of the tasks he is required to complete each day for work.  Individual is unable to be in areas where children frequent in the community.  Individual enjoys preparing and cooking food, cleaning, and helping others.

PROVIDER REQUIREMENTS: Individual requires constant visual supervision while in the community and workshop.  Individual may need verbal prompts to remind him of what he needs to do during the day.  Staff may provide verbal support in times of high anxiety.

TRANSPORTATION NEEDS: Individual requires a non-modified vehicle.

DIAGNOSES:  Mild Intellectual Disability, Impulse Control Disorder, Bipolar Disorder, Intermittent Explosive Disorder, ADHD, Fetishism, Pedophilia, Anxiety Disorder, Insomnia, GERD, Neuroleptic Induced Tardive Dyskinesia, Neuroleptic Induced Acute Akathesia, Pseudo seizures, Mood Disorder-NOS, Binge Eating Disorder and Insomnia, type 2 diabetes

HOUSING: Individual receives 24-hour supports within his home.

DAY PROGRAM/EMPLOYMENT: Currently not attending a day program or working at this time.  Individual periodically volunteers at local food banks in the surrounding area.
