DESCRIPTION OF INDIVIDUAL: Individual is a 27-year-old male. He has Autism, Tourette’s Syndrome, Apraxia, and Epilepsy. He is verbal but it is difficult for those who are not familiar with him to understand him. He is an elopement risk and needs constant visual supervision. He likes to be busy and move around during his day. He loves to ride in the car and go places. He can follow a schedule and does best when he knows his schedule.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-modified transportation
DIAGNOSES: Autism, Tourette’s Syndrome, Apraxia, and Epilepsy
HOUSING: Individual lives with his parent.
DAY PROGRAM/EMPLOYMENT: Individual attends an ADS program currently.
