DESCRIPTION OF NEEDED SERVICES: Individual is looking for Independent Employment Support. Individual works independently in the community 2-4x day per week. Job coach would need to check in monthly with the individual and her employer to go over work outcomes. Individual may need intermittent support if issues at work arise.

DIAGNOSES: Cerebral Palsy, Hypothyroidism, and ADHD.
HOUSING: N/A
DAY PROGRAM/EMPLOYMENT: Individual attends day program 3-4x per week.
