DESCRIPTION of NEEDED SERVICES: This individual is looking for an HPC provider to drive them to and from medical appointments, possible drop-in hours as well. Days/times and hours to be discussed with the family.

DESCRIPTION of INDIVIDUAL: The individual is 34 years of age and loves to sing, play bingo, color, and do word puzzles.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Mood dysregulation, paranoid schizophrenia, mild intellectual disability, diabetes mellitus.
HOUSING: Individual lives with her family in Madison, Ohio.
DAY PROGRAM: None
SUPPORTS NEEDED: Individual needs constant reassurance that no one is mad at her. Transportation to and from the community. Total assistance with purchases, Verbal reminders to use the bathroom.
