DESCRIPTION OF NEEDED SERVICES: The individual is at an increased risk of falls and skin breakdown and relies on caregivers to safely complete personal hygiene routines. He is looking for a shower chair that fits the recommendation provided by PT. A properly fitted bathing system is necessary to support health, welfare, and safety needs.
The shower chair should include the following features, as appropriate:
- Rolling base with locking casters.
- Durable waterproof frame.
- Adjustable footrests.
- Safety belt or positioning supports.
- Weight capacity appropriate for the individual.
- Commode opening and hygiene access.
- Adjustable armrests and/or swing-away components to assist with transfers.

DESCRIPTION OF INDIVIDUAL: The individual is a 64-year-old male who resides in a family home and requires extensive assistance with activities of daily living due to significant physical disabilities resulting from a traumatic brain injury and paralysis. The individual is wheelchair-dependent and experiences generalized muscle weakness, impaired balance, right-sided weakness, aphasia, and chronic fatigue. Assistance is required for bathing, dressing, toileting, grooming, transfers, mobility, and other personal care tasks.

TRANSPORTATION NEEDS: Modified Vehicle
DIAGNOSES: Significant physical disabilities resulting from a traumatic brain injury and paralysis. The individual is wheelchair-dependent and experiences generalized muscle weakness, impaired balance, right-sided weakness, aphasia, and chronic fatigue.
