DESCRIPTION OF INDIVIDUAL: Individual is an 18-year-old young woman who lives in Perry, Ohio. She enjoys going out into the community and especially loves anything to do with art. She loves painting and animals.

TRANSPORTATION NEEDS: Non-Modified
DIAGNOSES: 22q11.2 Deletion Syndrome (DiGeorge Syndrome), ADHD, Autism
HOUSING: Client lives in Eastern Lake County, will be receiving services in her home as well as perhaps outside of home for respite care in a facility. She lives with a Shared Living provider.
DAY PROGRAM/EMPLOYMENT: Client attends Mentor High School.
