DESCRIPTION OF REQUESTED SERVICES: 29-year-old living at home in need of Provider of Home-Delivered Meals. The intention is to increase his independence and self-sufficiency by managing his own meal preparation when he is home alone. He is need of vegetarian meals.
SEEKING AGENCY OR INDEPENDENT PROVIDER: No Preference.

DESCRIPTION OF INDIVIDUAL: Individual is bright and independent in some areas. Limited use of one side of his body, including his left arm. Vegetarian.
PROVIDER REQUIREMENTS: 7 meals per week; vegetarian preparation.

DIAGNOSES: Hemiplegia: left side; Cerebral Palsy; Seizure Disorder
HOUSING: Lives at home.
