DESCRIPTION OF REQUESTED SERVICES: Individual is looking for waiver home with roommates. He requires general supervision at home and in the community. He needs verbal reminders and encouragement to keep his space tidy and clean and may require some physical assistance with chores. Verbal to physical assistance with meal planning and prepping. Verbal reminders for ADL’s and hygiene routine. Monitor for signs of allergic reactions (nuts). Encourage appropriate social skills (not interrupting, redirecting when getting off topic). Must speak calm, quiet voice, and not yell.

DESCRIPTION OF INDIVIDUAL: 25 y/o male. He participates in Special Olympics and loves playing golf. He is interested in Asian culture and enjoys swords. He loves music and plays different instruments. He is very interested in various historical events and history. He also enjoys playing video games and watching videos. He has a history of verbal abuse from a previous provider, so provider should be conscious of this and speak to him in a friendly tone.
PROVIDER REQUIREMENTS: N/A – individual is self-med
TRANSPORTATION NEEDS: non-modified vehicle.
DIAGNOSES: Intellectual Disability, Autism, Bipolar Disorder, mild asthma, GERD, hypertension, obesity, ADHD (combined type), Impulse control disorder, Major depressive disorder, Oppositional defiant disorder
HOUSING: Currently resides in an apartment with his mom in Willoughby
DAY PROGRAM/EMPLOYMENT: Currently attends a day program in Concord, OH
