DESCRIPTION OF NEEDED SERVICES: 27-year-old female individual looking for 10 hours of HPC supports per week. She prefers hours on Mondays, Thursdays, and/or Fridays.

DESCRIPTION OF INDIVIDUAL: This individual can be described as organized, social, and outgoing. She enjoys taking care of and supporting others. She is an effective communicator and can directly express her wants and needs. She is her own guardian and has family support in place. She enjoys getting her nails done, going to the mall, going out to eat, playing basketball, and spending time with animals. She is independent with her self-care tasks/ADL’s/can independently administer her medications. She enjoys spending money and will need support to ensure her budget is considered. She likes to know about any changes in advance as she can become anxious. The client is working on filling her days to reduce impulsive decisions. She is hopeful to improve her decision making/build skills necessary to move into her own place.
PROVIDER REQUIREMENTS: Provider would need to be able to provide services as identified in ISP.
TRANSPORTATION NEEDS: Non-Modified vehicle
DIAGNOSES: Autism Spectrum Disorder, TIC Disorder, Asthma and ADHD.
HOUSING: Madison, OH with family
DAY PROGRAM/EMPLOYMENT: Not currently employed or attending a day program at this time.
