DESCRIPTION OF NEEDED SERVICES: Individual is a 38-year-old male looking for residential respite or HPC provider to care for him in his home, during times his guardian/shared living provider is out of town. This individual requires physical assistance with activities of daily living, meal planning and preparation, medication administration and transportation. Individual requires general supervision in his home and visual supervision in the community. Services may be required for up to 5 days.

DESCRIPTION OF INDIVIDUAL: Individual is a 38-year-old male who lives in his family home with his mother/guardian/shared living provider. He enjoys relaxing and watching tv while he is home. Individual communicates his wants and needs by using a combination of gestures, 1–2-word verbal phrases and a speech generating device. He thrives on following his routine and may become anxious if his routines are disrupted. Individual is mobile and does not require assistance to navigate but requires physical to total assistance with activities of daily living, meal planning and preparation, medication administration and transportation.
PROVIDER REQUIREMENTS: Provider must have Med. Certification 1.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Autism, PDD NOS, Anxiety and Epilepsy.
HOUSING: Services will be provided in his home while his mother/guardian/shared living provider is out of town.
DAY PROGRAM/EMPLOYMENT: Attends ADS program 5x/week at Patty’s People First.
