DESCRIPTION OF NEEDED SERVICES: Individual is looking for an HPC provider from 6:15-7:30am to sit with him while he waits for his ride to his Vocational Habilitation program. He is also looking for an HPC provider to be with him from 2:30-5:30p. This would be Monday-Friday.

DESCRIPTION OF INDIVIDUAL: Individual has limited verbal skills and has delayed speech. He uses 2–3-word phrases when talking. He likes it when someone talks to him in a quiet/calm voice and does not respond to yelling or a loud voice. He does well with a written schedule. He will jump up and down if confused, anxious, frustrated, or upset. He is protective of his food and will push you away or put his arm out of he thinks his food is being taken. He enjoys being on his iPad and will ask for it. Individual does not have any alone time and would need assistance with emergencies.
PROVIDER REQUIREMENTS: Certified in HPC.
TRANSPORTATION NEEDS: Non-modified
DIAGNOSES: Autism and a history of Grand Mal seizures. (Haven’t had one since 2000)
HOUSING: Currently lives in Wickliffe with his mom.
DAY PROGRAM/EMPLOYMENT: Currently attending a Vocational Habilitation program,
