DESCRIPTION OF REQUESTED SERVICES:
Looking for Residential Respite: 39-year-old male who resides with his parents looking for residential respite (flexible on hours/days but ideally 3-4 hours on M, W, F, and Sunday’s) at home and participate in community activities while ensuring health /safety needs are met. He uses a wheelchair for mobility and requires transportation in a modified-wheelchair accessible vehicle for community outings. Requires continuous supervision to maintain health and safety and assistance with all activities of daily living and mobility.

DESCRIPTION OF INDIVIDUAL: Male who enjoys car rides and watching movies/shows. He has a great big smile and relies on facial expressions, gestures, body language, and other nonverbal cues to communicate his wants, needs, and preferences.
PROVIDER REQUIREMENTS: Provider who can safely perform transfers and provide hands-on assistance throughout the duration of services.
TRANSPORTATION NEEDS: Modified vehicle.
DIAGNOSES: Angelman’s Syndrome, Severe Intellectual Disability, Seizure Disorder, Anxiety
HOUSING: Male resides with his parents in Concord.
