DESCRIPTION OF INDIVIDUAL: Individual is a 12-year-old male who spends one week with each parent on a rotating schedule. HPC is being sought in only one home due to natural support availability in the other.

Collateral information from caregivers and school documentation indicates the following information. He is non-verbal. He has a speech generating device that he may use with adult support at school. At home, they rely on non-verbal cues such as vocalizations and prompts.

The individual requires prompts and reminders to move throughout his routines and activities of daily living. Sometimes he will require multiple verbal prompts to complete a task. He has some noted issues with coordination navigating his school environment to best support him he has a 1:1 aid at school to ensure his health and safety.

He struggles with understanding cause and effect and will repeat unsafe behaviors, such as touching a hot stove, without regard for personal safety. He is not independent in the toileting process and wears pull-ups nightly. He requires prompts to use the toilet throughout the day; he will not use the toilet on his own and he does have accidents during the day if not prompted and at night. He has been to the emergency department 5-6 times in the past year due to putting things in his ears and nose. He would not be able to protect himself from abuse nor report any abusive behavior aimed at him. He needs assistance obtaining a snack and opening packaging.

When he is upset, he will bite himself, scratch himself, throw himself on the floor, cry, spit, scream, grab or attempt to bite others who are assisting him.  He bites his knee when frustrated and will pick at his nose or bite his lip until bleeding. These behaviors are often without a clear antecedent.

Caregiver reports that he will grab a woman’s breast and attempt to squeeze/twist the breast when upset. He has required redirection at home when in a common area when he has touched his own private parts.
