DESCRIPTION of NEEDED SERVICES: Fun 70-year-old male is in need of HPC and HPC transportation for social opportunities within the community 2-3 days per week for approximately 3-4 hours. He enjoys going to the Broadmoor Basketball games, going to the movies, and would like to attend the local Senior Center. Staff would provide visual and verbal supervision while in the community. Individual can walk with walker but prefers using his wheelchair.

DESCRIPTION of INDIVIDUAL: Individual is a fun, social 70-year-old male with a good sense of humor and resides in a congregate home in Mentor, Ohio
PROVIDE REQUIREMENTS: This provider will deliver ISP services as documented in the ISP. Staff to provide visual and verbal supervision while in the community.
TRANSPORTATION REQUIREMENTS: Individual uses a transportation wheelchair.
DIAGNOSES: Individual is diagnosed with Moderate ID, IED, Hypertension, non-insulin-dependent diabetes, and Hyperlipidemia. Has dexterity issues in hands.
HOUSING: Individual lives in a congregate home in Mentor with 3 other individuals.
DAY PROGRAM/EMPLOYMENT: Individual attends a day program Mentor, 3 days per week.
