DESCRIPTION OF NEEDED SERVICES: Individual is a 20-year-old male looking for an HPC provider to assist him with activities of daily living, community integration and safety skills. He lives with his mother and sister and is interested in having a provider work in the home for a few hours per day. He is non-verbal and does not use a communication device but can make simple choices with gestures. He requires total assistance with completing his hygiene routine, toileting/changing depends, meal preparation, dressing and transportation. Individual requires auditory supervision in the home, visual supervision in the community and close and constant supervision when near streets and/or parking lots.

He requires total assistance with completing his hygiene routine, toileting/changing depends, meal preparation, dressing and transportation. Individual requires auditory supervision in the home, visual supervision in the community and close and constant supervision when near streets and/or parking lots.
PROVIDER REQUIREMENTS: DODD certified.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Autism Spectrum Disorder

DAY PROGRAM/EMPLOYMENT: Looking to attend an ADS program.
