DESCRIPTION of NEEDED SERVICES: 40-year-old male individual looking for a HPC service. Individual would need around 5 hours a day, unless attending day program. Individual will also need transportation to access the community.

DESCRIPTION of INDIVIDUAL: Individual is a 40-year-old male with Autism, PKU, Mild ID. Individual prefers to have a routine. Individual does have some behaviors including both physical and sexual. Individual is friendly and easy-going. He likes to be outdoors doing activities.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Autism, PKU (phenylketonuria) and Mild ID
HOUSING: Individual currently resides with her parents in Mentor, Ohio.
DAY PROGRAM: N/A
SUPPORTS NEEDED: Visual supervision in the community and assist with purchases. Total assistance with meal planning and physical assistance with meal preparation. Total assist with medication administration. Review safety skills. Transportation to and from community outings. Assistance with hair care, nail care, shaving, bathing/showering, etc.
