DESCRIPTION OF INDIVIDUAL: 45-year-old male seeking Vocational Habilitation Program to attend one day per week, preferably—up to 2 days per week, potentially—on days he isn’t involved in Group Employment. He has experience with janitorial work in Group Employment and previous Vocational settings, seeking ample opportunities to earn money during his time as well as opportunities to meet people and make connections with peers. Independent with all ADLs. May need reminders to keep conversations with peers appropriate and to respect boundaries with peers and staff.
PROVIDER REQUIREMENTS: NA
TRANSPORTATION NEEDS: Will also require NMT.
DIAGNOSES: Tourette’s Syndrome, Major Depressive Disorder, Anxiety, Intermittent Explosive Disorder—all controlled currently by medication. Mild Intellectual Disability.
HOUSING: Currently lives in a congregate setting in Wickliffe with 2 other individuals.
DAY PROGRAM/EMPLOYMENT: Group Employment with Deepwood CES 2 days per week—on Tuesday and Friday.
