DESCRIPTION OF REQUESTED SERVICES: 22-year-old female looking to explore a new day program. Individual is interested in ADS and attending 5 days a week. She is new to Lake County and is interested in making new friends and being involved in the community. Individual would need transportation to and from the day program. She is interested in starting a new program in the beginning of January.

DESCRIPTION OF INDIVIDUAL: She can be difficult to understand at times but will repeat herself if you ask. She is sensitive to loud noises, lights, and emergency vehicles(sirens) as they can trigger seizures. She is aware of when she has a seizure, and it can be very painful for her. She needs assistance with uneven surfaces. She requires visual supervision in the community and at the day program.
PROVIDER REQUIREMENTS: NMT, ADS, Med. Certification (Takes PRN seizure meds)
TRANSPORTATION NEEDS: Non-Modified Vehicle
DIAGNOSES: She is diagnosed with Polymicrogyria, Cerebral palsy, Autism, Epilepsy, Developmental disabilities.
HOUSING: She lives with her parents in Painesville.
DAY PROGRAM/EMPLOYMENT: Currently attending Person Centered Services
