DESCRIPTION OF REQUESTED SERVICES: 26 y/o male in search of Monday-Friday ADS program. He would prefer NMT morning support with the same agency providing ADS but is open to utilizing another provider if that is not an option.

DESCRIPTION OF INDIVIDUAL: Individual is typically happy, well-mannered, and compliant with requests. He thrives when there are daily routines (specific foods, activities, locations, and even specific furniture) but also enjoys spending time with his parents and HPC staff exploring the community, walking in the park, or engaging in any water-based activities. He has a base vocabulary content but typically communicates through scripting, humming, or typing on his iPad.
The following are potential challenges for the individual. When new providers don’t know the individual’s communication cues, it can lead to frustration. Historically, when the individual has been frustrated, he will flail his arms and legs or bite his own hand. He has not harmed anyone else when frustrated and his behavior responses have never resulted in a need to implement a behavioral support plan. A second potential challenge is that the individual has a sleep disorder, which can impact his activity level during ADS Programming. He may prefer to rest rather than engage in an activity or community outing.
PROVIDER REQUIREMENTS: Follow all services as outlined in the OISP.
TRANSPORTATION NEEDS: Non-Modified vehicle
DIAGNOSES: Autism Spectrum Disorder and Sleep Disturbance.
HOUSING: He resides with his parents in Willoughby.
DAY PROGRAM/EMPLOYMENT: In search of new ADS, Monday-Friday; he prefers not to work.
