DESCRIPTION OF NEEDED SERVICES: Individual is looking for a day program to attend during the week. Looking to attend full-time Monday-Friday. Individual has some challenging behaviors around social boundaries, time management, and redirection from technology. Having some challenges, he benefits from verbal redirection and verbal prompts, visual schedules, social stories, and a routine to be successful.

DESCRIPTION OF INDIVIDUAL: Individual is a 24 y/o male. He enjoys boats, trains, and car rides. He uses an iPad with ProLoQuo2Go to communicate. May become physically aggressive towards others and staff with too many redirections and prompts – given time for processing, physical distance, and patience, individual can be successful. Triggers to anxiety and/or anger include communication barriers, loud noises, fireworks, gun noise, a change in routine, or not having his iPad/if it’s not working.
PROVIDER REQUIREMENTS: ADS certification, medication administration (oral/topical). Staff should be trained on crisis prevention, deescalation techniques, intervention techniques, Aegis, etc. due to history of aggressive behaviors.

DIAGNOSES: Autism, Moderate ID, Anxiety, OCD, Apraxia, Pervasive Developmental Disorder NOS, Motor and Oral Hypotonia
HOUSING: Resides in a home in Eastlake, OH
DAY PROGRAM/EMPLOYMENT: Currently attends a program in Cleveland, OH
