DESCRIPTION of NEEDED SERVICES: This individual is a 31-year-old male who resides in Kirtland. He is looking to attend a day program with non-modified transportation up to 5 days per week. He enjoys his routine at the program but will go out in the community on preferred outings.

DESCRIPTION of INDIVIDUAL: This individual is safest within visual supervision at day program and close and constant in the community, especially going out to watch trains! He is non-verbal but will use gestures and physical cues (pointing or will bring you item) to let you know his wants/needs. He will also nod/shake his head to yes/no questions. When at program, he carries a container of sensory items that he uses to self-soothe.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non modified vehicle
DIAGNOSES: Severe intellectual disability, autism, and seizure disorder
HOUSING: He resides at home with his mother in Kirtland.
DAY PROGRAM: He previously attended a day program in Mentor, 5x per week.
SUPPORTS NEEDED: He is looking for assistance on socialization with peers, going out in the community, and having activities to do throughout the day to keep him busy.
