DESCRIPTION OF REQUESTED SERVICES: 32-year-old female is looking for a HPC provider that will take her out in the community on Thursdays and Fridays. She is looking for about 3-5 hours each day. She would be available from 10-3 each day.

DESCRIPTION OF INDIVIDUAL: She is verbal and is able to make her needs known. She enjoys shopping, talking, going to the zoo, and other parks. She can get anxious around aggressive males, dogs, or crowded places. She does require supervision on stairs and curbs due to blindness in left eye.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: non modified vehicle.
DIAGNOSES: Mild ID, Developmental Delay, Anxiety, Mild CP, and Hydrocephalus secondary to Meningitis with VP Shunt, Legally Blind L eye, Near & Far Sighted
HOUSING: Lives with her mom in Concord.
DAY PROGRAM/EMPLOYMENT: Works at Laketran Fleet
