DESCRIPTION OF REQUESTED SERVICES: Individual is in need of a repair for the wooden platform that connects her outdoor lift to her home. The wood is beginning to shift, causing the platform to pull away from the door, creating a gap and lip for her wheelchair to go over. Additionally, the wood is pulling the door frame away from the house, creating a gap between the home and the frame of the door.

DIAGNOSES: Dystonia, Aspirations, Chronic Pulmonary, Gastrostomy tube dependent, Dysphagia, Oropharyngeal Phase, Spastic Quadriplegic Cerebral Palsy, Excessive Salivation, Truncal Hypotonia, Hyperkinesia, Hip Dysplasia, Neuromuscular Scoliosis of Thoracolumbar region, Hypoxic Ischemic Encephalopathy, Urinary Incontinence, without sensory awareness, Constipation, Ineffective Airway Clearance, Hyper Kinetic Disorder and difficulty regulating her body temperature
