DESCRIPTION OF REQUESTED SERVICES: Female client seeking ADS program with NMT. Would prefer to have a part-time schedule with interest to increase days over time.

DESCRIPTION OF INDIVIDUAL: This person is a 23-year-old female who lives in Concord with her parents and brother. This individual can communicate verbally but has a delayed response and will repeat what you are saying. She has reading and writing skills. She is able to complete most grooming tasks with verbal prompts and is able to ambulate independently. She requires 24-hour care with auditory supervision at home and visual supervision in the community. She may need to be re-directed from grabbing, pushing, and does sometimes throw items. She has OCD and one of the tics she displays is frequent somersaulting. She will typically do this no matter the environment she is in and does not review if her surroundings are safe first. She benefits from reminders to twirl in place when somersaulting is observed. She has a variety of interests including listening to music and singing. She likes plush dolls, Disney Princesses and female characters. With support, she likes to participate in leisure/recreational activities in the community like bowling, adapted ice skating, zoo, museums, Sky zone and going to get French fries.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-Modified Vehicle

HOUSING: Lives in Concord with family- Mom is shared living provider.
DAY PROGRAM/EMPLOYMENT: Searching for part time ADS with NMT supports. Prefers to have a mix of community involvement/time at the day program site.
