DESCRIPTION of NEEDED SERVICES: Individual is seeking a Remote Supports provider to assist with problem-solving/troubleshooting things that may come up while they are home alone.

DESCRIPTION of INDIVIDUAL: Individual is 25 and lives at home with family. He is able to communicate verbally and understand communication and is fully ambulatory. He is independent with most tasks once he has attempted them a few times but needs help to read and understand labels if he decides to cook or manage unanticipated things that come up throughout the day.
PROVIDER REQUIREMENTS: Remote Supports and ATQ certification
DIAGNOSES: Autism, ADHD
