DESCRIPTION OF REQUESTED SERVICES:
Looking for an ATN/ HPC Transportation provider for mostly evening and weekend activities. Individual needs rides to and from seasonal activities with Special Olympics, LCBDD Rec programs, and Bowling. Other activities could be TBD but planned in advance.

DESCRIPTION OF INDIVIDUAL: Individual is a 21-year-old male. He communicates well verbally with others and is typically well understood. He is friendly and sociable. He has won metals through Special Olympics and is a dedicated Giant Eagle employee. He takes transportation via his day program and uses Lake Tran for his work.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-modified transportation
DIAGNOSES: Autism, Metabolic Disorder, Nutrition Deficiencies
HOUSING: Individual lives with mom in Eastlake.
DAY PROGRAM/EMPLOYMENT: Individual attends a day program M-F in Painesville. He also has a job at Giant Eagle.
