DESCRIPTION OF REQUESTED SERVICES: NMT, 2 trips per day/3 days per week, Mondays, Tuesdays, Wednesdays, to and from home in Mentor and Packij Solutions, 38363 Western Parkway, Unit 4, Willoughby. Earliest arrival time to the program is 7:30 am and latest pick-up is 1:15 pm.

DESCRIPTION OF INDIVIDUAL: The individual is a female, 34 years old, has no medical or behavioral concerns, and is independently mobile. She is verbal and speaks English with no vision or hearing impairments. It is important to be on time and communicate with her mother should an interruption or change in transportation occur.

HOUSING: Individual resides in the family home in Mentor
