DESCRIPTION OF INDIVIDUAL: Individual is a 62 female who only speaks using Napoli sign language. While this is a strong barrier, her HPC staff who get her ready in the morning, and her ADS staff can communicate with her and you with any information needing to be shared. She is very friendly and uses a cane for walking.
PROVIDER REQUIREMENTS: N/A
TRANSPORTATION NEEDS: Non modified vehicle
DIAGNOSES: severe mixed bilateral hearing loss, arthritis, vitamin D deficiency, Gout, Hypothyroidism, GERD and episodes of dizziness.
HOUSING: Transportation will be needed from her home in the morning in Mentor Ohio to her ADS provider in Cleveland (14031 Puritas Ave Cleveland, OH 44135)
DAY PROGRAM/EMPLOYMENT: Her day program Intra-National Home Care LLC. Begins at 9 and ends 3. Pick up from ADS would be around 3:20
