DESCRIPTION OF REQUESTED SERVICES: Individual is looking for a new Voc. Hab/ADS Program. Individual would like to attend day program Monday-Friday. Individual is interested in Voc. Hab 3 days a week and ADS 2 days a week.

DESCRIPTION OF INDIVIDUAL: Individual is a 22-year-old male living in Mentor with his mom and shared living provider. Individual has a behavioral add on. Individual may become frustrated when he needs help or does not like something. He does enjoy being active. He enjoys cooking, being outside, collecting sea glass, and loves sports cards.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-Modified vehicle
DIAGNOSES: Autism, Anxiety, and OCD
HOUSING: At his family home in Mentor
DAY PROGRAM/EMPLOYMENT: Attending Hilltop Supported Living
