Schedule: 10:30am-1:30pm Monday, Wednesday & Friday
ADLs: Individual requires verbal reminders to complete personal hygiene tasks thoroughly (bathing, oral care, hair care, deodorant). Assistance completing household chores (dishes, laundry, trash)
Medication reminders: Individual requires text reminders to ensuring he takes medications daily

DESCRIPTION OF INDIVIDUAL: Individual is very independent and prefers a provider that encourages him to do as much as he can on his own. Individual can becoming frustrated if he feels like provider is underestimating his capabilities to do things independently.

DIAGNOSES: Autism Spectrum Disorder, Pervasive Developmental Disorder, Mild ID, Acid Reflux, High Cholesterol, High Triglycerides, Sleep Apnea, Depression, Anxiety Disorder NOS, Iron Deficiency, PTSD, Obesity, Seizure Disorder
HOUSING: Individual lives alone in Madison, only requires services in home
