DESCRIPTION OF REQUESTED SERVICES: 47-year-old male individual looking for wheelchair accessible 24/7 congregate setting. Client can live in Co-Ed setting or all male setting. Needs total assistance with all living skills (hygiene routine, eating, medication assistance, transportation, etc).
SEEKING AGENCY OR INDEPENDENT PROVIDER: Agency provider in congregate setting.

DESCRIPTION OF INDIVIDUAL: Client is looking to move from his current ICF setting in Madison. He enjoys being part of the group and doing tasks to the best of his ability. Although he needs total assistance with all household chores/self-care tasks he likes to feel that he can do more and have staff give him positive feedback when he tries. His favorite task to help with is sweeping with a broom after mealtimes. He is verbal, although he is hard to understand by those who do not know him well. He needs total assistance to propel his wheelchair from place to place. He cannot feed himself without physical assistance. He likes to make choices and have his options explained to him. He has an APSI guardian. He has a tablet that he likes to use for entertainment and values photos of himself & family. He is described as someone with a great sense of humor who can make anyone laugh- He enjoys having staff support to write down his ideas and thoughts, loves music, and it is important to him to stay busy.
PROVIDER REQUIREMENTS: Client requires total assistance for administration of medication. Needs total assistance for all transfers, uses Hoyer with two person transfers.
TRANSPORTATION NEEDS: Modified Vehicle Required
DIAGNOSES: Severe Intellectual Disability, Cerebral Palsy, Spastic Quadriplegia 2nd to CP, Multi Flexion contractures of arms and legs, nystagmus, periodontal disease, seizure disorder, mild hyperopia, urinary incontinence, Vitamin D deficiency, anemia, ileus, constipation, depression, osteoarthritis.
HOUSING: Looking for wheelchair accessible home
DAY PROGRAM/EMPLOYMENT: Attends program in Geneva 5 days per week.
