DESCRIPTION OF REQUESTED SERVICES: Individual is looking for HPC in a female only waiver home. She requires physical assistance with curbs, stairs, uneven surfaces, and walking, physical assistance to find new and fun activities to do in the community and provide access to those activities, verbal to physical assistance to complete routine chores around the home, total assistance with heavy cleaning, transportation to community events, grocery, and medical appointments, physical assistance in the shower, physical assistance with changing her Depends and ensuring cleanliness, offer verbal reminders when she is consumed in an activity or before leaving the house to use restroom. total assistance to administer all medication, encouragement to exercise, eat healthy, and wear her glasses, track her calories in her notebook and encourage her to do so, keep record of all transactions and receipts on a ledger and provide total assistance with making purchases, provide physical assistance with warm/hot liquids/beverages and food items.

DESCRIPTION OF INDIVIDUAL: Individual is a 58 y/o female. She enjoys doing crafts/coloring, drinking coffee, and reading from her Bible. She is motivated to continue her health journey and continue losing weight/maintain a healthy weight. She values having a routine and knowing what to expect.
PROVIDER REQUIREMENTS: Med certified staff required, needs total assistance with meds
TRANSPORTATION NEEDS: non modified
DIAGNOSES: Down’s Syndrome, Moderate Intellectual Disability, Schizoaffective D/O, Anxiety, Hypothyroidism, left eye-legally blind, gallbladder surgery, heart murmur, recurrent c-diff infection, high cholesterol, onychomycosis, constipation, dry mouth.
HOUSING: Currently lives in Mentor, OH. Would prefer location close to Mentor, OH.
DAY PROGRAM/EMPLOYMENT: Attends a day program in Mentor 5 days/week.
