DESCRIPTION OF REQUESTED SERVICES: Seeking Private Duty Nursing for assistance while attending day program and possibly in the community. Individual requires total assistance for all daily and hygiene needs. He has limited ability to assist with transfers but can operate his wheelchair independently. Individual would need assistance with medicine and nutrition via G-tube, trach care, and incontinence assistance. If care is provided overnight, he would also need assistance with setting up and operating a C-Pap. 485 completed, suggesting up to 40 hours/week of home nursing. NTI will need to be completed, as well as approval process.
