DESCRIPTION of NEEDED SERVICES: Individual is looking for a Vocational program or ADS program Tuesday, Thursday and Fridays.

DESCRIPTION of INDIVIDUAL: Individual is a 36-year-old female. She is verbal but can sometimes be hard to understand. She enjoys bowling, going to karaoke, and participating in Special Olympics. She is a hard worker and enjoys doing some yard work. She likes playing games, watching TV, and helping take care of her sister.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Mild Intellectual Disability, Irregular heartbeat, Anxiety, Bipolar disorder II, Depression, Gingivitis, Onychomycosis, Plantar warts, Asthma, Arthralgia, Seasonal allergies Shortness of breath, Binge eating disorder; Impulse control disorder, History of constipation; Pre-diabetes
HOUSING: Currently lives with sister and one other roommate in Wickliffe.
DAY PROGRAM: Attends ViaQuest 5 days a week.
SUPPORTS NEEDED: Requires auditory supervision in the home and visual supervision at all times in the community. She has a history of stealing from stores or others. She needs verbal assistance to complete hygiene and cleaning tasks. Individual needs physical assistance to prepare food. Individual needs a provider that can assist with accessing the community at least weekly.
