TRANSPORTATION NEEDS: non-modified vehicle.
DIAGNOSES: Pyloric Stenosis, Hyperthyroidism, Depression, Post Traumatic Stress Disorder, Fetal Alcohol Syndrome, Moderate Intellectual Disability
HOUSING: Person currently resides in a waiver home with 1 roommate in Wickliffe.
DAY PROGRAM/EMPLOYMENT: Employed part time in Mentor, OH
