DESCRIPTION of NEEDED SERVICES: Kind, thoughtful and energetic 56-year-old female is in need of HPC in the community along with transportation Monday-Friday after 3:00pm approximately 3 days during the week and Saturday and Sunday flexible. She enjoys going out to dinner, shopping, going to the movies, church and supporting her with personal errands. Staff provide visual and verbal supervision while in the community.

DESCRIPTION of INDIVIDUAL: Individual is a 56-year-old female who resides in a congregate setting in Mentor, Ohio
PROVIDE REQUIREMENTS: This provider will deliver ISP services as documented in the ISP. Staff provide visual and verbal supervision while in the community.
TRANSPORTATION REQUIREMENTS: Individual is mobile and can transport in a non-modified vehicle.
DIAGNOSES: Mild intellectual disability, generalized anxiety, bi-polar disorder, chronic dysthymia, depression, topical eczema, seborrhea, mild astigmatism, asthma related allergic rhinitis, hypertension, hypothyroidism.
